Nakano Kōichi Kanshū: Keirin Ō follows the career of a professional cyclist navigating the strictly regulated Japanese velodrome circuit. The narrative is one of discipline and progression, focusing on the daily grind of a trainee as they move through the ranks of the national association. The atmosphere is clinical and professional, capturing the high stakes environment of a sport where every fraction of a second is monitored by both officials and spectators. It serves as a digital representation of the professional values championed by its namesake, Koichi Nakano, during the height of his influence as a world champion sprinter.

Gameplay consists of menu driven management where the player selects specific training regimens to improve a cyclist’s stamina, speed, and mental resolve. During the racing phases, the game uses a side scrolling perspective with large, detailed sprites to depict the tactical jostling for position behind the pacer. The mechanics involve managing a finite energy bar while using the draft of other riders to launch a final sprint at the correct moment. It supports up to four players in a sequential mode and utilises a battery backup to save season progress and athlete data. This title is the first in a series of Keirin simulations that includes the sequel Nakano Kōichi Kanshū: Keirin Ō 2 on the same hardware and the later Nakano Kōichi Kanshū: V-Keirin on the PlayStation.
